Medical History Form
Please arrive 15mins prior to your scheduled appointment time to complete our medical history form.
Appointments & Cancellations
Please contact us to book an appointment at a time convenient for you. If for any reason you must cancel your appointment, we require 24 hours notice or a cancellation fee may apply.
Payment
Payment is required at each appointment. We have HICAPS for direct billing of your health insurer and cash, credit card, and EFTPOS facilities available.
Find Us
We are located at 12/247 David Low Way, Peregian Beach. We are about 100 meters north of Peregian Village, and parking is available down the left-side driveway (behind the building). Public Parking is also available at the next driveway along. If driving south on the David Low Way towards us, you will need to use the roundabout at Peregian Village to make a U-Turn as there is no Right Turn facility into our carpark. If you’re not driving, there is a bus stop just a few minutes walk away.
